Health Requirements for new and current students.
St. Juliana Catholic School follows the State of California’s requirements for student immunizations. All admitted students will need to submit the following health-related documents:
- Report of Health Examination for School Entry
- TB-Risk-Assessment
- Oral Health Assessment
- California Immunization Record
All admitted students, including TK/K and transfer students, are required by California State law to have the following immunizations:
- Diphtheria, Tetanus, and Pertussis (DTap, DTP, Tdap, or Td) - 5 doses
- Polio (OPV or IPV) - 4 doses
- Hepatitis B - 3 doses
- Measles, Mumps, and Rubella (MMR) - 2 doses
- Varicella (Chickenpox) - 2 doses
TDAP BOOSTER FOR 7TH GRADE ENTRANCE
A Tdap booster for whooping cough (pertussis) is required for all students entering 7th grade. (Tdap = tetanus, diphtheria, and pertussis). This requirement can be met by receiving one dose of Tdap vaccine on or after the 10th birthday.
