Medication

School personnel will not administer any medication to any student, at any time, unless the parent request form and physician’s statement is on file. No student is allowed to keep medication in the classroom or on his/her person during the day. This rule is for the safety of all students. If a child must take any prescription

medication at school, that medication must be sent to the school in the container received from the pharmacy and must have on its label in English the following information:

  1. Child’s name

  2. Name of doctor prescribing the medication

  3. Frequency

  4. Dose

  5. Date

  6. An appropriate measuring device must accompany all liquid medication.

Students’ medications are to be kept in the school health room. All medication must be picked up prior to the last day of school.
