Medications 
Aspirin and other medications will not be furnished to any student by school personnel. Over the counter medications must be
  • in original containers
  • accompanied by a consent form, obtained from the school health room and signed by the child's physician and parent,
  • provide details regarding the amount, method and time schedule medications are to be administered.
If students are required to take prescribed medication during school hours, parents must deliver the medication to the health room

Medicine must kept in the health room. No medication may be kept in classrooms unless permission has been obtained from the administration. Students should be capable of self-administration of their own medication under supervision of school personnel.

Please print and fill out the Parent/Guardian and Authorized Health Care Provider Request for Medicine form when a parent/guardian is requesting administration of medication at St. Juliana school.

Students’ medications are to be kept in the school health room. All medication must be picked up prior to the last day of school.
